Windows Pharma Collection Kits

All supplies necessary for the collection samples are provided in the collection kit. In addition, all collection tubes, transfer tubes, and laboratory requisitions have been pre-labeled for your convenience. Please note that the majority of the collection kits can be used for any visit and are not subject specific. Please see your laboratory manual for specific kit information. Supplies included in each kit include:

Pre-labeled collection tubes and cryovials
Needle, hub, and tourniquet
Band-aid, gauze, and alcohol prep pad (if permitted in shipping by country)
Plastic transfer pipettes
Pre-labeled laboratory requisition

Supply Order Form

Kits must be ordered as needed. Kits are not automatically resupplied!! Collection kits and supplies can be ordered in the following manner:

Fax the "Supply Request Form" found in the lab manual to 512-439-5000. Please complete all fields.
Complete and submit the online Supply Order Form. Please complete all fields.

Study-specific laboratory manuals are created for each study and should be used when placing orders for kits and supplies.
